According to New York State Education Law, a physical exam is required for all new entrants to the school and at grades 2, 4, 7, and 10. These physicals should be performed by your own physician as each child has the best assurance of proper attention to his or her health needs when examined by his or her own doctor.

Please have your physician complete the physical exam form and return it to the school nurse.

Yearly Mandated Screenings:

The New York State Department of Education mandates the following yearly screenings:
• Vision – All new entrants in grades K, 1, 2, 3, 5, 7 and 10
• Hearing – All new entrants in grades K, 1, 2, 3, 5, 7, and 10
• Scoliosis – Grades 5-9
